Have you assigned a custom cover before? Does the file have embedded cover art?
Embedded cover art is implicitly updated each time Mixxx is loading a file. If you have removed the embedded cover art from the file since it has been imported into your library, it will disappear the next time you access the track in Mixxx. Custom cover art selected by the user should always be preserved and never replaced by Mixxx!

What type of file is affected?
Copying and attaching the mixxx.log file after this happened would be helpful.
Mixxx dont remove cover art from file, it just show an empty column in track (library) window.
and it didnt reloaded if try to use load metadata from file 'command'.
cant remember how i managed get it back.
this happend a mp3 files which are embeded cover art or cover art is in jpg file.
These files have been in library, and i reset grid bmp info. and re analyzed.

Unfortunately I'm not able to reproduce the behavior you described, neither for files with embedded cover art nor for files with a custom image file selected as cover art.

For further analysis we need an excerpt of the log file when the issue occurred and a detailed step by step description of the actions you did and which results you actually observed. If the issue only affects some files then an example file could help.
i am talking track(library) window column covert art. not talking files.
have'nt found a logic when it not show or lose cover art from column cover art.
I am trying to re-analyze my songs, as told in forum.
When i dropped about 200-300 songs, there now after analysis many, plenty of songs
which cover art column is now empty.

This week I also did many small improvements in the dlgtrackinfo layout and major changes in the actions of the popup menu (accessed clicking on the cover art).

- changing window title - using “Artist - Title” instead of “Title”- drawing all fields with fixed height (25px)- fixing bugs when the user resize the window- using cover placeholder as a qwidget instead of qgroupbox (causing weird behavior on Windows7)…

UNSET COVER
- changing the cover location to default_cover instead of use coverId=-1. It avoids Mixxx to keep searching for a cover (after unset).
RELOAD COVER
- now it uses the searching algorithm. It will try to reload the cover as normally we do (firstly looking in the id3tag and then in the trackdir).

I also added an icon for each action of the popup menu [4]. The icons which I pushed are generics, it means that maybe they are not ideal yet (design). However, for future changes we just would have to overwrite the current files.
